Dimension based payment form

1. You can use a multi-tabs element to contain the sections of your form. Click the Add Question & Element button on the element toolbar, search for tab, and click the Add button on the Tab card.

2. Click the 'Add a panel' button on the element toolbar.

3. Click the tab's initial label 'Enter text' and update the label.

Note: Clicking the tab area outside the label only selects the tab, to edit the label, click the 'Edit text' (pencil) button on the tab's toolbar.

4. Repeat the steps to add other panels: Personal details and Employer details

5. To add questions or elements inside the tab's panel, click the tab. If you accidentally click the tab's label, click the 'Add text' (pencil) button to deactivate the text editing. Click the Add Question & Element button on the left toolbar or the 'Add element' button on the element toolbar.

6. Search for numeric, and click the Add button on the Numeric input field card.

7. Click the question's text and update it.

8. To deactivate text editing, click the 'Edit text' (pencil) button on the element toolbar. Because we are going to create the same numeric type of question for the width, press Ctrl+C or Cmd+C on MacOS to copy the currently selected numeric question. Press Ctrl+V or Cmd+V on MacOS to paste it after the current selection.

9. Update the question's text to Width (W), and click the 'Edit text' (pencil) button to deactivate the text editing.

10. Press Ctrl+V (Cmd+V on MacOS) again to paste a question for Measure, and update the text.

11. Click the 'Edit formula' button on the element toolbar and enter

12. To add line break, click the add line Append line break button at the bottom left corner of the selection. Alternatively, while not editing a text, press Enter.

13. To select the line break, click the small circle at the right end of the line break.

14. Add a choice drop-down question. Search for a choice and click the Add button on the Single-choice drop-down card.

15. Click the 'Edit drop-down choices' button on the element toolbar to edit the choices' label.

16. To type a superscript, while editing the text, click the 'More formatting options' (down arrow) button on the text editing toolbar, and choose Superscript. To turn off the superscript, click the Superscript button again.

17. To use the unit options in a formula such as Total amount payable, add values to the drop-down options. To add a value to an option, click the 'Set choice value' button on the option's toolbar. Click Apply.

18. To add the total amount payable question, search for currency and click Add button on the Currency input field card.

19. Click the 'Edit formula' button and enter

Click Apply.

Note: A choice question returns an array with a single member for a single choice and multiple members for a multiple choice question. To convert a single member array to a numeric, use the VALUE function.

 20. To add personal and employer details, follow the above similar steps using different types of questions, such as single-line text input field for first name and last name and email type for the email address field.

21. Click the Title & Settings button on the left toolbar. Enter the title of the form and the description.

22. To allow the user to download the submitted form as a PDF file, enable the option 'Allow the user to download the submitted form as a PDF file. Click Apply.

23. Click the Publish button on the top bar.

We use the above steps to create this form template. To edit this form template, after signing in to your account, open the following URL on another tab, and click the 'Edit this template: ....'

Payment requires a paid subscription. To add payment, click the Payment button on the left toolbar

Click the Choose this button on the Stripe Card Payment card.

Click the Grant button on the Account card. You need to create a Stripe account, if you don't have one. Follow the instructions.

On the Payment action and currency tab, choose your payment currency and type of payment (default: Charge).

On the Formula for totals, fees and optional payment, click the Pick variable button in the Total value field.

Move the mouse on top of the question that calculates the total amount payable, and click the variable button.

To apply the payment settings, click Apply.
